How to Complete the Form

  • Use the form below and fill out all required fields.
  • Include your travel destination, departure date, and any special airline or country requirements if known.
  • Submit the form as soon as possible — ideally at least 30 days before travel — to allow time for processing and any needed vaccinations or exams.

Once your form is submitted, our team will contact you to schedule a travel health exam and finalize your certificate.

  1. Was your pet’s rabies vaccine given at another clinic?
    If your pet’s rabies vaccine was not given at Pet Medical Center of Boca Raton, you must obtain the rabies vaccine certificate from the other practice and email it to info@petmedicalcenterboca.com.
  2. Rabies certificate(s) must include:
      • Veterinarian’s signature
      • Microchip number
      • Vaccine manufacturer
      • Product name
      • Serial or lot number
      • Lot/serial expiration date
      • Date administered
      • Date vaccine expires

    If your pet’s rabies vaccine was given more than one year ago, please send the last two rabies vaccine certificates.

  3. If any additional vaccines were done at another facility, email those records to info@petmedicalcenterboca.com.
  4. Some countries accept digital endorsement of International Health Certificates. Others require you to purchase a shipping label, meaning the destination country requires an ink signature and embossed seal. The endorsed certificate will be returned by mail, and you must provide a prepaid, pre-addressed return label at the time of submission. Pet Medical Center of Boca Raton is not responsible for creating your shipping label. It is the owner’s responsibility to complete this prior to the appointment if required by the destination country.
  5. If your travel destination is an ORANGE BANNER country, please ask Pet Medical Center of Boca Raton to email you instructions for obtaining the shipping label.
  6. After we research your pet’s travel requirements, additional information may still be needed for the health certificate. Every country has different requirements related to laboratory testing, vaccines, and medications. We will advise you on next steps, but it is still the pet owner’s responsibility to stay informed by checking official resources.

  7. USDA Endorsement & Processing Disclosure: Once the International Health Certificate has been submitted to the USDA for review and endorsement, it is no longer under our control. All review, endorsement, approval, and processing timelines are determined exclusively by the USDA. We monitor the submission status and remain available to assist when possible. We cannot expedite, influence, or guarantee the USDA's processing times. We expect the endorsement to be returned before your scheduled departure date; however, this timeframe is not guaranteed and remains subject to the USDA's review process. Please note that the USDA endorsement system is a secure portal accessible only by the veterinarian who submitted the health certificate. Our support staff cannot view the status or retrieve documents from this system. Once the veterinarian receives the endorsed certificate, we will promptly notify you and provide your finalized travel documents as soon as they become available. By proceeding with the international health certificate, you acknowledge and understand that USDA processing times are outside of our control and may vary without notice. We appreciate your patience and understanding throughout this process.
  8. High-Risk Rabies Country Travel & U.S. Re-Entry Disclosure
    If your pet is traveling to or returning from a country designated by the CDC as a high-risk country for dog rabies, additional requirements may apply. These requirements are especially important because dogs returning to the United States from high-risk rabies countries are subject to specific CDC entry regulations.

    For dogs returning to the United States from a high-risk rabies country, the CDC requires specific documentation to be completed before the dog leaves the United States. A USDA-accredited veterinarian must complete and submit the Certification of U.S.-Issued Rabies Vaccination form for USDA endorsement prior to departure. Please note that the Certification of U.S.-Issued Rabies Vaccination form is a separate document from the International Health Certificate and is required in addition to the health certificate for eligible travel scenarios. Preparation, review, and USDA endorsement of this additional form require separate veterinary documentation and processing and are subject to an additional fee.

    If this required documentation is not completed and USDA-endorsed before your pet leaves the United States, re-entry options may be limited. Dogs returning without the required documentation may be subject to additional CDC requirements, including the foreign-vaccinated dog entry process, which may involve further documentation, restrictions, and additional steps before entry is permitted.

    We strongly recommend reviewing all CDC requirements before travel to ensure your pet has the necessary documentation before departure. Travel requirements can change, and pet owners are responsible for ensuring compliance with all applicable CDC and USDA regulations.
